PARENT, athlete & VOLUNTEER INFORMATION
The safety and integrity of the football and cheer program is of utmost importance. We ask all those who want to play, coach or volunteer to please review and complete the following forms and checklists.
athlete requirements
Each football player & cheerleader must have the following items in order to start July 15th:
• Printed copy of birth certificate
• completed 2026 piaa physical or Pop Warner Physical Form - PIAA Download
• copy of 2025-2026 school year report card
• completed forms 1&2 below
